How this website handles information about you, in plain terms.
This policy, and the one it is not
This policy covers this website: what it collects when you fill in a form or read a page, who that information reaches, and what you can ask us to do about it.
It is not our Notice of Privacy Practices. That is a separate document, required under HIPAA, covering the medical records the clinic holds about patients and the rights you have over them. Nothing here changes it. If you would like a copy, ask us at your appointment or call (614) 759-1176.
What we collect
When you request an appointment. The form asks for your name, a phone number, an email address, whether mornings or afternoons suit you better, and anything else you want to tell us. That goes to the clinic as an email so we can call you back. It is not saved on this website and there is no database behind the form.
When you book through the calendar. Choosing a time on the booking calendar creates the appointment with Acuity Scheduling, who run it for us. What you type there — your name, your email address, a phone number and the time you picked — is stored on their system as well as reaching us, which is different from the form above. Acuity is named again below.
When you use the text form. It does not send us anything. It writes a message in your own phone's messaging app and leaves it there, unsent, for you to read and send yourself. Until you press send, nothing has left your phone.
When you simply read a page. Our host records the ordinary technical details every web server records: your IP address, your browser, the page requested and the time. That is used to serve the site and to keep it secure.
Please do not send us medical details through this website. Email and text are not secure, and the forms here are for arranging a time to speak, not for describing symptoms. We will cover the clinical side on the phone or in person.
Who else sees it
Acuity Scheduling. They run the booking calendar, so an appointment you make there is held on their system. They are a business associate of the clinic under HIPAA, which is the contract required of anyone handling patient information on a practice's behalf.
Google. The clinic's email runs on Google Workspace, so an appointment request passes through Google on its way to us. Google has signed a Business Associate Agreement covering this, which is the contract HIPAA requires of anyone handling patient information on a practice's behalf.
Cloudflare. They host the site and pass your request to it.
Advertising and analytics companies, but only if you agree. If you accept them, cookies from Google and Meta tell us which pages are read and help us measure advertising. You can refuse, and refusing costs you nothing — every page works the same either way.
We do not sell your information, and we do not share it with anyone else except where the law requires it.
Cookies
A few cookies are needed for the site to work at all, and those are always on. Everything else — the analytics and advertising ones above — waits until you say yes.
You are asked once, when you first arrive. You can change your mind at any time using the cookie settings link at the foot of any page, and your browser can clear or block cookies independently of anything we do.
Texts and calls
If you give us your number, we will use it to reach you about your appointment. Message frequency depends on what you have asked us for and is generally only a handful around a visit. Message and data rates may apply.
Reply STOP to any text to stop receiving them, or HELPfor help. Stopping texts does not affect your care, and we can still reach you by phone.
How long we keep it
An appointment request lives in the clinic's email for as long as we would keep any enquiry, and becomes part of your record only if you go on to become a patient — at which point the Notice of Privacy Practices governs it, not this policy. Server logs are kept briefly, for security.
What you can ask for
You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Call(614) 759-1176 or write to us at the address below and tell us what you would like. We may need to confirm who you are first, and some records we are required to keep by law.
This site is meant for people in the United States, and it is not intended for children under 13.
Changes, and how to reach us
If this policy changes we will update the date at the top of the page. Material changes will be flagged on the site rather than made quietly.
